INTRODUCTORY PACKS AND PROGRESS REPORTS:

Introductory Packs

Before coming to your first appointment, please download an introductory pack so we can work better together to help your pet. We have specific introductory packs for specific kinds of conditions, or your pet may need a general pack - please contact Helen directly if this is the case or you are not sure which category your pet falls into.

I apologise in advance that some parts of the introductory packs are a bit long and boring - the lawyer had a lovely time merging my obligations under both the physiotherapy and veterinary laws (as even though I am NOT a vet, because I work on animals, I must respect their laws!) Please note also that all introductory packs have consent forms - it would be a marvelous help if you could bring your consent form already signed to the first appointment! Please also read the 'preparing for the appointment' form that the lawyer made me write so you know what to expect - the FAQs is also helpful for those wanting to make sure they come prepared!

Progress Reports
Please note that once you start physiotherapy, you will need a Progress Report, so feel welcome to check them out before coming - I will explain exactly what to do with them at the appointment, don't worry!

It really helps if carers send in a Progress Report each Monday to update me on your pet's physiotherapy. This enables us to better schedule appointments, because we know how your pet is going without having to bring them in "just in case" (which would add to your bill unnecessarily). If your pet is progressing as expected, I can reassure you that this is the case, whilst if your pet is having any difficulties, we can get on top of the problem quickly. It has also been my experience that those owners who keep a written record of what homework they have done and when, have better outcomes with their pets, hence the Progress Report was born!

You will get a hard copy of the Progress Report form at your first appointment, but many people have asked for an electronic version. Please 'save as' the electronic version on your computer (e.g. Ally Nicholson Progress to 030509) and email it to me for the fastest possible response:

Please remember: if you don't leave your contact details VERY CLEARLY on your Progress Report, the reality is I won't be able to contact you about your pet's progress!! With so many Progress Reports to reply to and so many kids hiding phone numbers under the couch, if I have to ring the hospital to get your phone number, it all goes into very very slow motion... Please help me help you: ALWAYS INCLUDE YOUR CONTACT DETAILS - preferably two phone numbers so we don't play phone tag all week - EMAILS WORK BEST!
